What are GLP-1 weight loss injections?

The GLP-1s (Glucagon-Like Peptide-1 receptor agonists) are a class of drugs, that have been used for nearly twenty years in the management of diabetes. Published research in that time has shown the drugs to be safe and effective for weight loss, as well as diabetes. 

The drugs work by mimicking a naturally occurring gut hormone called GLP-1 and act on GLP-1 receptors in three ways to reduce appetite and increase fullness after eating.

Ignoring the old-fashioned weight loss drugs, Orlistat and Reductil which were ineffective and sometimes harmful, the drugs we now prescribe have different names but are all types of the same class of drugs called GLP-1 analogues or medications.

Broadly speaking, Yes. They have been used for nearly twenty years in diabetic patients with a low-risk of harm. However, there are some risks and side effects which are discussed in the FAQs and will be discussed with you before you start the medication.

Anyone with a body mass index over 27 kg/m2 can be a good candidate for weight loss injections, but the best candidates are those who are ready to embrace lifestyle and dietary change as well.

The weight loss outcomes in the published studies are impressive, with patients losing an average of 20% of their body weight with Mounjaro. In all of the studies the participants were given intense dietetic support and encouraged to exercise too. The medications are unlikely to be as effective as demonstrated by studies without significant dietary and lifestyle change too.

Achieving long-term weight loss is a marathon, not a sprint. Most side-effects are soon after starting the medication, they decrease in severity within a short space of time, unless the dose is increased. So it’s probably best to be patient, trust the process and follow the expert advice.

Within hours of taking the medication, you will be aware of the medication controlling your appetite and lessening your interest in food. Within a few days weight loss should follow and within a few weeks increased energy, smaller clothes sizes and so on.

The injections are generally well tolerated, but they are not without side effects. The severity of the side-effects will not only vary from person to person but also with the specific medication and dose.

As with side-effects, the medications are generally safe with longer term more serious complication of the medication being rare, but the following have been seen with the medication.

The published research studies that assessed the GLP-1 injections for weight loss ran for between 9 months and two years. To get similar outcomes to the published results individuals will need to take the medication for a similar time frame. In the studies when the medication was stopped, hunger returned with some weight regain.

In our opinion, patients will probably need to use the medication intermittently for the long-term. Some people will choose to take the medication continuously, as is the case for diabetic patients who are prescribed GLP-1 drugs to control their blood sugars and weight.

In all the published studies of weight loss injections, patients had intense dietary support too. It is likely that patients who follow a diet while taking medication will get better weight loss than those who do not, but it does not need to be a specific diet. Streamline’s dieticians will work with you to find a diet that suits you which may be plant-based, low carb, high protein or more individualised.

The injections work by controlling appetite. As anyone who has dieted knows, dieting is difficult because the body responds to a diet, as it would to a famine, increasing appetite and the drive to eat. It is beyond almost everyone to fight this basic instinct indefinitely, without medication or surgery.

Because the injection mimics a naturally occurring intestinal hormone, it is usually well tolerated, rarely a cause for concern with other illnesses and uncommonly interacts with other medications, with the exception of diabetic medication.

Nevertheless, before starting the medication, anyone with chronic illness, weight related illnesses, other conditions such as cancer or taking multiple medications should seek professional advice.

The Streamline clinicians who assess each person before the medication is prescribed will advise whether or not a consultation with an bariatric physician is needed.

Surgery is normally reserved for patients with higher body mass indexes, typically over 35kg/m2. Although in studies, surgery has been proven to be more effective than medication, some higher BMI patients will successfully lose weight without surgery.  

Although surgery with Streamline is normally straightforward, with complications being uncommon, it is still a much more major undertaking than using medication. Not unreasonably for many people surgery is not an option they want to consider and for this group of people the medication can be a very effective alternative. Many patients will want to give medication a chance before deciding to have surgery.
