The procedure works by removing the upper portion of the stomach, where most of the hunger hormone ghrelin is produced. With lower ghrelin levels, your appetite naturally decreases—making it easier to control portion sizes and avoid overeating.
Gastric sleeve surgery works by dramatically reducing the size of the stomach, so it can no longer hold large amounts of food between meals.
During the procedure, the stomach is stapled vertically and reshaped into a narrow tube—just over one centimetre in diameter—reducing its capacity by around 60–70%. As a result, patients feel full much sooner and are unable to eat large meals, helping them adopt healthier portion sizes and support sustainable weight loss.
To be eligible for a gastric sleeve most patients will need to have a BMI over 35, but with illnesses related to weight such as diabetes or high blood pressure surgery may be appropriate at lower BMIs. You can calculate your BMI using our online tool.
To support your success as with any weight loss surgery, patients need to be prepared to work hard and follow the advice given by your clinical team.

The Gastric Sleeve, known as a sleeve gastrectomy, is one of our most popular weight loss procedures where the capacity of your stomach is reduced by 80%. This transforms your stomach from a large grapefruit size to a narrow tube the size of a banana.
The gastric sleeve, also known as a laparoscopic sleeve gastrectomy used to be the first part of a two-stage operation, the duodenal switch, a malabsorptive operation, which was usually reserved for patients with very high body mass indexes, greater than 60 kg/m2. But with time, it became clear that many patients had successful, sustained weight loss, with the gastric sleeve alone.
Increasingly surgeons recognised the sleeve’s reliability and increasing numbers of patients chose the sleeve over the gastric bypass recognising it was easier to live with in the longer-term. The sleeve gastrectomy became increasingly popular, from the early 2010s onwards. Since then, more and more long-term outcome studies have been published, supporting its effectiveness as a weight loss operation and also a metabolic operation for illnesses such as diabetes.
The length of time the gastric sleeve surgery takes to perform depends on the patient and the surgeon. For an easy sleeve, the surgery will take 45 to 60 minutes however, for more difficult procedures this can take much longer, up to two hours. The important thing to remember is, it’s not important how long a patient spends under anaesthetic; it is much more important the surgery is completed carefully to minimise the risk of any complications

Recovery from gastric sleeve surgery, is usually quick, with many patients feeling better the morning after surgery and then every 12 hours after that, feeling much better. The first night after surgery can be tough as you may experience nausea, generalised abdominal pain however, this usually passes after 24 to 48 hours after surgery. Most patients are fit for discharge on their second day and by this time are mobile and self-caring. Most patients take two weeks off work and are thereafter back to most of their usual activities, but strenuous exercise is not recommended until around 6 weeks after surgery but everyone is different and Streamline take this into account.
